GJ Diagnostic Labs may remove, modify or otherwise change any content, including that of third parties, on or from this Site. GJ Diagnostic Labs may impose limits on certain features and services or restrict your access to parts or the entire Site without notice or liability. GJ Diagnostic Labs may terminate your use of the Site at any time at its sole discretion.

These terms apply exclusively to your access to and use of the Site and do not alter the terms or conditions of any other agreement you may have with GJ Diagnostic Labs or its parents, subsidiaries, or affiliates. Medical tests specified in the health checkup package are conducted as a screening procedure for early detection.

The health checkup package cannot be split between two or more people.

Some medical tests are center-dependent; tests may vary from center to center.

Some medical tests may not be conducted if the person receiving the tests reports with any contraindications medical or otherwise.

CONDITIONS OF REPORTING

1. It is presumed that the specimen belongs to the patient named or identified in the test request form.

2. A test requested might not be performed for the following reasons:

A)   Specimen quantity insufficient (inadequate collection/spillage in transit)

B)   Specimen quantity unacceptable (haemolysed/ clotted/ lipemic etc.)

C)   Incorrect Specimen type

D)   Test cancelled either on request of patient or doctor or because of incorrect test code, test name or specimen received. Reference may be provided to a new Accession No. under “COMMENT” if the specimen has been reaccessioned for a different test. It is expected that a fresh specimen will be sent for the purpose of reporting on the same parameter(s) if required.

3. A test requested might yield “INVALID RESULT” for various technical reasons and this response will appear against the test name followed by a detail comment at the end of the report.

4. A report that carries a “PRELIMINARY” status signifies that results are yet to be reported for one or more of the test or else, as is the case with many microbiology tests, a “FINAL” culture, identification or drug susceptibility result might be pending. In such case, the descriptor “RESULT PENDING” column and will be replaced by the results whenever the latter are ready. The reports will when completed, acquire a “FINAL” status.

5. If the collection date was not stated in the test Requisition Form, the same will not be printed on the report.

6. Any query pertaining to this report may be directed to the Customer Care group at this laboratory on the phone no. listed on the front page.

7. Some tests are referred to other laboratories to provide a wider test menu to the patients. The detail of the laboratory where the sample was referred to, can be obtained from Customer Care Group.

When you use our Website/App, we collect and store your Personal Information. Our primary goal in doing so is to provide a safe, efficient, and customized experience to our Users. This allows us to provide services and features that most likely meet your needs, and to customize our Website/App to make your experience safer and easier. Importantly, we only collect Personal Information about you that we consider necessary for achieving this purpose.

 Additionally, we also collect the location data of our (i) service provider personnel/phlebotomists when the website/app is running in the foreground or background of their mobile device through GPS, IP address, to track their trips to our Customers location and keep them updated about our Phlebotomists real-time location status (ii) Customer from their mobile devices through GPS, if they enable us to do so, as to reach their serving location accurately and also to enhance your use of our apps, including to improve pick-up locations, enable safety features, and prevent and detect fraud.

 In general, you can browse the Website/App without telling us who you are or revealing any personal information about yourself. To fully use our Website/App, you will need to register using our online registration form, where you may be required to provide us with your contact and identity information and other Personal Information as indicated on the forms throughout the Website/App and complete the Registration Process. Once you give us your Personal Information and initiate the Registration Process, you are not anonymous to us. Where possible, we indicate which fields are mandatorily required and which fields are optional. You always have the option to not provide information by choosing not to use a particular service or feature on the Website/App, unless we require it so.

from time to time, to reveal general statistical information about our Website/App and visitors, such as number of visitors, number and type of services purchased, etc;

All credit card transactions happen using Secure Server Software (SSL) for 128-bit encryption through third-party gateways and the Company plays no role in the transaction, except for directing the customers to gateways or the relevant webpage. Accordingly, the Company shall not be responsible or liable for any loss or damage due to any disclosure whatsoever of Personal Information or any other information collected by the gateways or such Website/Apps.
